Fleur De Lis Family Healthcare is dedicated to providing you with the highest quality, cost-effective medical care. We will bill most insurance carriers as a courtesy to our patients; however, it is the responsibility of the policy holder to know what services are covered and not covered by the insurance healthcare plan and what the accountable portion is for each service. We always suggest to the policy holder to verify with the insurance company that our Nurse Practitioners are participating providers in the healthcare plan.

If the patient will not be responsible for payments of services, payment arrangements must be made with the responsible party prior to the visit. It is our policy to collect payments upon service, including any deductible, copayments and self-pay payments. If your insurance has a copayment, your insurance carrier requires that we collect it from you at the time of service.

New insurance plans are added and the insurance provider lists and handbooks aren't always updated and reflected in a timely manner. Even if your employer-based medical insurance is not listed, there is still a good chance we are in-network with them. Many plans are owned by larger companies and it may not be easy to recognize.
